I feel from how well known and influential they are, Psycho is #1, with The Shining, Alien and the Exorcist taking the next spots. After that you could argue a hundred scenes, but I would have to go with the Jaws opening with Tina's death in A Nightmare on Elm St as an honorable mention.

Nothing from 'The Thing' (1982)? The chest-muncher eating the doctor's arms off! The first husky dog transforming in the kennel! 'Psycho' shower scene would be my #1, as well. Masterful idea.

There was a scene in Alien that was cut out of later versions of the movie, I thought was worse than the chest burst scene. Parker tried to destroy the xenomorph, but Lambert froze in fear, and it went to town on both of them. It was the later scene where Ripley was running, and she stumbled into that compartment. The carnage left behind was beyond disturbing.

3-5 is debatable for me.
My #2 is Halloween 1 when the detective shoots Michael Myers and in an instant, he vanished. That scared the begeziz out of me. lol
My #1 all time horror movie scene is the jail cell scene from Gothica with Halle Barry. The camera and cinematography were unmatched. I was clearly shaken, even in front of my wife!

The exorcist was my first trauma followed by F13 and NOES. Samarah traumatized me as a teenager.

The only scary scene of the Exorcist is when the 7 foot tall chest of drawers animated itself and goes after the mother . That is really creepy but everything else is just fake and stupid , especially the 180° head turn .

Honestly that scene from Signs is probably one of the most realistic & FULLY viscerally felt out of ALL mentioned. Especially if you're from a heavy "unknown/UFO/cryptid" activity area with CENTURIES (as in BEFORE airplanes & blimps, etc were a thing) of documented, photographed &/or recorded events that multiple & dozens of people to whole strings of townships witness where in the past 12-15yrs even local law enforcement & surrounding county/city/township govts have made official statements regarding various events stating that THEY don't know wtf & aren't getting pertinent (or ANY) answers from either the state/fed govt, nearby airfields (CVG is 15 min drive & 3 smaller airfields are even closer,) OR the Military as we're in the most SW corner of Ohio so Wright-Patt isn't terribly far & we do get very low military fly overs by Army & Air Force doing "weird" maneuvers during training, but also air transport of military equipment, etc & ALL local authorities make statements openly & ON THE RECORD, including on NextdoorDowntown, FB & through community email chains. THAT is terrifying even if it's our own military - PARTICULARLY if it's our own military or other unknown countries militaries, really. That'd definitely be in my top 5, possibly № 1, especially in today's social-political climate, unnecessary war AND mass fear mongering. Joaquin/Leaf Phoenix played that to absolute PERFECTION. Positively bone chilling, w/ amazing direction & fabulous camera work for sure. M. Night Shyamalan is incredibly gifted in eliciting visceral reactions, even when the ending of the story is anticlimactic & unsatisfying, that uneasy, unsettling feeling lingers.
